EUO deposition title page

I had to do "examination under oath" via conference call and was wondering if anyone had any info on how you may have done the title page for this type of depo?  THANKS!!

Also, make sure you put telephonic somewhere or via telephone if the witness wasn't there in person.  As a notary public, you can only swear in the witness if they are there before you in person, at least in my state.  There should be a notary public with the person, wherever they are, swearing in the person, and then you should have the notary's information to include on the EUO or depo who they were sworn in by.

Same except instead of DEPOSITION OF JOHN JONES I put EXAMINATION UNDER OATH OF JOHN JONES, and the cert page says examination under oath instead of deposition. :-)
