I like this question because it all comes down to personal preference and style and the "right" is whatever the reporter chooses!  I think not punctuating at all would make it more readable.  But if the answer was actually "something else" then it becomes a more important part of the question.  In that case, then I'd put a comma after constant.

That being said all of the answers already given have justification, it's just how you think it best reflects how the question was asked and answered.

Seems like there are really two questions here, one about what she described, the other about what it was like.  Probably not the popular choice, but I'd punctuate it this way:

Did she ever describe whether or not the bleeding was spotting?  Or was it constant or something else?

Agree with Fahrenwide, boils down to reporter preference and style.

I hate commas. Flusters up the transcript. For me, the norm is after if and when clauses. It flows so nice without, why fix it?

I`ve learned to put a comma before or when it`s a sentence or new thought after. So I would put it after spotting. It also depends how they said it, like if there was a pause, too.

I looked in Morson`s index under: commas - in a series of questions, Rule 13 >>Caution B indicates if conjunction is used after each item in the series use no commas to separate the items with the question mark at the end.
